In addition to your name and phone number, please provide the pole number (a 3-digit or 4-digit number found on the lower portion of the pole) or the street address nearest the pole. Please also describe the nature of the problem that you have observed such as complete outage, dim or flickering light, light on during daylight hours, etc. Requests for repairs received by Wednesday are typically repaired by the end of the same week.
